Surgery Consent Form
Before any surgery, your surgeon will walk you through the recommended procedure, what to expect during recovery, and any potential risks, and will answer any questions you have. This consent form confirms that you understand and approve the planned procedure. Take your time reviewing it, and reach out if anything is unclear before you sign.
